We are excited to have you as a patient and look forward to helping you live your healthiest life. Before your visit you can save time by completing the Patient Registration Forms below.
Please bring the following with you to your appointment:
- Driver’s License or ID Card
- Insurance Card
- If you are uninsured and/or applying for the Sliding Fee Scale, please bring 1 month of most recent household income

We are required by law to maintain the privacy of your Health Information, provide you information about our legal duties and privacy practices, inform you of your rights and the ways in which we may use Health Information and disclose it to other entities and persons.
Accepted Insurances
We work with a wide range of health insurance plans, ensuring you have access to care you can trust. In additional to public health insurance programs (Medicare and Louisiana Medicaid), we accept a variety of HMO, PPO and Medicare Advantage Plans. If you do not see your insurance listed below, contact our Billing Department at 878-5171 for a more inclusive list.
Standard Charges
We know health care costs, billing and insurance can be confusing, and we strive to address patient questions with clarity and compassion. We have created a list of standard hospital charges to allow patients to look up anticipated charges for health care services. While it is impossible to know ahead of time which services will be needed to address each patient’s specific medical needs, these tools provide an estimate of anticipated charges.
